R17 Workforce Development – Persons With A Disability – LMTA UPDATE

Responsibilities
This process is performed by an authorized official.

Purpose
Workforce Development – Persons with Disabilities (WD-PWD) services are Career and Employment Information Services (CEIS) that may provide job search, work exposure, and job maintenance supports for individuals with barriers to finding and maintaining competitive employment with their current skills, behaviors and experience. The purpose of WD-PWD is to assist individuals with a disability obtain or maintain labour market attachment by developing job search and work skills to address barriers to employment, and exposure to new or alternative occupations in the labour market.

Pre-conditions
The individual:

  • is registered in Mobius with federal data fields completed; and
  • has a disability
  • has an Integrated Case; and
  • has a complete Assessment identifying barriers to employment; and
  • is Labour Market Destined: and
    • Labour Market destined implies the individual is ready, willing and able to commit themselves to pursing employment service(s) that will enable them to actively seek and accept employment.
  • has an active Service Plan

NOTE: Disability for LMTA purposes refers to a physical, mental, sensory, intellectual or learning impairment which, in interaction with various barriers, may hinder labour market participation.

Trigger
Initiate the WD-PWD process when an individual is requesting employment services and has satisfied all of the pre-conditions for WD-PWD.

Process

  1. Add the Plan Item “Attend Workforce Development-PWD” to the Service Plan
    1. Enter the Expected Start Date and End Date:
      1. Expected Start Date (consult the Products and Services Catalogue and ensure the dates matched) is the date the individual is accepted into WD.
      2. Expected End Date is the date the individual is expected to complete all of their components of WD-PWD.
  2. WD Referrals (see Refer a Plan Item in Mobius User Guide). An Authorized official is to complete the following fields in Mobius to refer and link the individual to an appropriate WD service location:
    1. Ensure the Referred to Provider section in Mobius is completed.
      1. Enter Provider name:
      2. Enter the Expected Start Date.
      3. Search the Programs and Services Catalogue for a Start Date.
      4. Select the appropriate listing. Save.
  3. Edit Plan Item (see Edit Plan Item in Mobius User Guide):
    1. Enter the Actual Start Date when the individual is a WD participant (status = in progress). Save.
  4. WCB Acknowledgement
    1. Have the individual sign the WCB Acknowledgement section of the signature page, which is an addendum to the TESR form. The individual’s signature is required to participate in WD. For Records Management purposes, record the Mobius ID # and Name in the top right hand corner of the last page if signed. Signing the WCB Acknowledgement is a precondition of attendance in the above-mentioned employment services. In the event an individual refuses to sign the WCB Acknowledgement, they will not permitted to participate. Signed records are retained in accordance with the appropriate Records retention schedule.
    2. In Notes section of the Service Plan Home Page with a Subject line of “WCB Acknowledgment” document the status of the WCB Acknowledgment i.e. received or refused.
  5. Record Service Management Updates
    1. Capture Service Management updates (e.g. contact or attempts to contact individual or additional information relevant to an Individual’s progression through services) in Notes section of the Service Plan Home Page with a Subject line of “Service Provider Update”

      NOTE: This step is repeated to ensure the individual’s progress in WD-PWD services is assessed regularly and progression through WD-PWD services is documented in Mobius Notes.
  6. WD-PWD Completion
    1. Edit Plan Item “Attend Workforce Development-PWD”:
      1. Enter Actual End Date, which is the last day the individual is a WD-PWD participant. Use the date prior to the individual being employed or the last date the individual attends WD-PWD, whichever applies.
      2. Select “Complete” for Outcome. Save. OR
      3. Select “Incomplete” for Outcome if the individual walks away or has been removed from WD-PWD before completing the services. Save.
    2. Save Plan Item.
  7. 90-Day Follow-up* (see Performance Measures Policy)
    1. Contact the individual for a follow-up to ensure accountability of this service and the intended outcomes are achieved.
    2. Add Plan Item “Conduct Follow-up” and Category “90” days:
      1. Enter the Expected Start Date, which is 90 days following completion of the WD-PWD Services or the last date the individual attends WD-PWD services.
      2. Enter the Expected End Date, which is the date follow-up is/is to be completed. Save.
    3. Edit Plan Item “Conduct Follow-up”:
      1. Enter the Actual Start Date, which is the date when contact actually starts.
      2. Enter the Actual End Date, which is the date when contact with the individual is made and follow-up is completed, or when contact has ended/discontinued after reasonable efforts have been made to complete the follow-up.
      3. Close the Plan Item “Conduct Follow-up” as “Complete” once follow-up is completed. Select “Incomplete” if contact with the individual has ended/discontinued and follow-up is not completed.
    4. Enter in the Notes section of the Service Plan Home Page the specific information about the results of the contact with the individual.
    5. After the follow-up, update the Employment screen and any other relevant demographic/administrative data as appropriate.
      * Follow-up is to be conducted at around the 90-day mark following the last day of active service.
  8. Monitoring
    The WD authorized official is to monitor and ensure the proper closing of all Plan Items and completion of the LMTA required data in Mobius.
