R03.2 Service Plan Closure - LMTA UPDATE

Description

In order to have relevant and reliable information it has been determined to have Mobius users close inactive or completed service plans.

When there is no active service delivery, the service plan with the Individual should be closed.  This is relatively a discretionary process based on the Individual but the following are some guidelines to assist staff.

Active Service delivery is when the Government of Alberta (GoA) or product provider on behalf of the GoA is providing programs or services to the Individual.  The service delivery is finished when the Individual has finished accessing the programs and services they were seeking assistance with.  This can be direct or passive completion of the service plan. 

Direct is when the Individual has completed the program/service and obtains employment. Direct is also when the Individual indicates they no longer would like to receive services/programs and/or benefits.
Passive is when there has been no contact from the Individual for more than 90 days and they have not accessed any other program/service offered by the GoA or product provider.

For example:

  • If they have accessed a specific service such as resume services and obtained employment, the service delivery is completed.
  • If the Individual has come to access funding and approval for an exposure course, once the Mobius User has followed up, the delivery is completed and plan can be closed if not accessing any other service/program.
  • Active service delivery is if the individual has ongoing employment services, such as attending Transition to Employment Services, or meeting with a Career Employment Consultant then they are considered to be in active service delivery and the plan remains open until they have obtained their goal (i.e. employment)

Process

To close a service plan. 

  1. Ensure all plan items are complete with Actual dates entered and any relevant notes are captured.
    1. All items should have a status of complete, incomplete (started but did not complete) or canceled (did not start). 
  2. Any sub-goals should also be completed with an updated status.
  3. Confirm that the service plan status says Active and then update the plan by editing the service plan and choose the appropriate outcome.
  4. At the top of the service plan home page choose close service plan and the relevant reason. 
  5. The Mobius User when closing can choose to provide a closing statement in the comments section that summarizes the activities taken in the service plan.
  6. Add any relevant notes to the Integrated Case Notes page to ensure anyone else interacting with the Individual knows what has occurred last.

    NOTE:
    In conducting service management and the Mobius User is unsure if an Individual’s service plan is still active as there is no activity on the file, review the Integrated Case notes. If there is no indication, the best practice is to contact the last staff person who was working with the individual to determine if the individual’s case is active (still receiving services or follow- up) or dormant and requires closure.
