1. From the individual / family Common Outcome Plan Page, select the Activities tab from the Navigation Bar
2. Select Services sub tab
3. Select the list action menu
4. Select Delete
   
   

Notes:

• Delete a service only if it was entered in error, NOT if it is ended

5. A modal will open with a confirmation message
6. Select ‘yes’ to delete (service will no longer appear)
7. Select ‘no’ to go back to service screen
