Purpose

Pathway is now recorded in both the Needs Identification (NI) and Common Holistic Assessment (CHA). Pathway is in two categories; Recommended Pathway (what the staff recommend) and Chosen Pathway (what the individual chooses).

Procedure

Anchor Point: Needs Identification (NI) in Common Outcome Plan, Anonymous NI, or CHA

1. Select the Pathway (Supported Self Service or Case Management) you are recommending to the Individual
2. Discuss with the Individual the level of assistance for the recommended pathway
3. In Chosen Pathway, select the Pathway for the level of assistance the Individual chooses (Supported Self Service or Case Management)
   
   
   
4. The last saved Chosen Pathway, regardless if it is from NI or CHA, will appear on:
   1. Person context panel
   2. CCISD Case context panel
   3. COP context panel
