Enhanced Needs Identification

1.3 Enhanced NI-Needs and Pathways Section

Purpose

The Enhanced Needs Identification (NI) combines the previously existing Initial Contact-reporting purposes with the Needs Identification information collection under domains. The Enhanced NI can be done anonymously then attached to a registered individual, or simply completed and ‘filed’ for reporting. The Enhanced NI can also be done from an existing registered individual within the Common Outcome Plan (COP).

Procedure

Anchor Point: Anonymous Needs Identification or Needs Identification in Common Outcome Plan

  1. Select the drop down toggle beside the Needs and Pathways heading

    Primary Need
  2. In the first drop down, select a Primary Need category
  3. In the second drop down, select one program that was primary to the individual
  4. Only 1 primary needs can be chosen

    Note:

    The second drop down under primary need will contain only the programs associated to the category chosen in the first box under Primary Need



    Secondary Needs
  5. Select as many programs under Secondary Needs as are relevant to the conversation with the individual

    Note:

All category fields will be expanded on the screen. Secondary needs are a duplicate of the Primary Need categories and programs



Pathway and Pathway Rationale

  1. Select the checkbox corresponding to the pathway recommended to the Individual
  2. Select the checkbox corresponding to the pathway chosen by the individual
  3. In the Pathway Rationale, enter why the pathway was recommended, as well as why the chosen pathway was selected (if different than recommended)

    Note:

    An NI contains the same fields regardless if it is Anonymous or within a Common Outcome Plan, and is completed following the same processes for completing the domain, needs, pathways, outcomes and next steps for the New Needs Identification


Case Management: Not Available

This is a checkbox that has been put in for future use. Right now it has no meaning or reporting impacts. Please do not check it off. You still recommend a pathway and the Individual chooses a pathway.

   

Mandatory Fields

  • Only one Primary Need can be selected
  • Secondary Needs are not mandatory as the Individual may have come in with only one need, which is accounted for in the Primary Need
  • Only one Recommended Pathway can be selected
  • Only one Chosen Pathway can be selected
  • At least one Outcome must be selected, but multiples can be selected

    Note:

    Domains are not mandatory but must be completed if NI is on a registered Individual, or if you suspect the Individual may register from an Anonymous NI
