R02. Assessment - LMTA UPDATE

R02.2 Service Needs Determination (SND) Assessment - LMTA UPDATE

Description
The SND assessment is a consultation between the individual and a Mobius User to determine what their presenting needs are and then through the dialogue what their identified needs may be.  It is to ascertain what programs and services the individual is seeking and which ones would be appropriate to offer and/or refer to.

Process:

  1. An individual is registered in Mobius and has an active Integrated Case, this includes ensuring their education and employment history is updated.
  2. Mobius User meets with the individual and asks why they have come to speak with them.
  3. Mobius User records their answer based on the 6 pillars (Career Services, Employment Services, Information Services, Skill Development, Financial Support and Non –Financial Support).
    1. The most urgent need is selected as the primary need and for some individuals there may be secondary as well as a third need recorded as the Tertiary need.  
  4. During the dialogue the Mobius User can affirm the reason the individual is requesting services and/or assess other identified needs based on the 6 pillars.
    1. The individual during this dialogue will be informed about the programs and services available to him/her to assist in meeting their needs.
  5. Notes are entered in the Notes section recapping the outcome of the conversation, the reason the individual came and what the Mobius User interprets as the identified needs from the assessment.
  6. Based on the assessment and identified needs Mobius User determines if the individual is labour market destined by choosing yes/no in the drop down options under notes.
    1. Labour Market destined implies the individual is ready, willing and able to commit themselves to pursing a training program and/or employment service(s) that will enable them to actively seek and accept employment. 
  7. Based on the conversation with the individual, the Mobius User then selects using the 6 pillars what needs have been identified and chooses the primary need and any further needs just as in step 3a above.
  8. Mobius User then saves the assessment, updates the outcome and chooses one of the following options in the drop down menu with the accompanying date:
    1. Complete Status- when the SND is finished (notes, labour market determination, identified needs recorded).
    2. Incomplete Status- when the individual chooses to leave the conversation or refuses to participate.


Once the assessment is complete begin working on a service plan. This may include conducting a more extensive assessment such as an employability assessment to meet the individual’s need(s) based on the SND.
