Anchor Point: Assessment tab in Common Outcome Plan

1. From the individual / family Common Outcome Plan Page, select the Assessment tab from the Navigation Bar
2. Click on the assessment hyperlink
   
   
   
   Note: The system will open and show the Assessment Page
3. Select the History tab
4. Select the List Actions Menu
5. Select Delete Assessment
   
   
6. Select Yes
   
   
   
   Notes:

• ONLY supervisors have the authority to delete an Assessment
• A CHA should only be deleted if it was entered on the Individual’s outcome plan in error
• Once deleted the CHA will be removed from the CHA list and is not retrievable
• If the assessment (CHA or NI) being deleted is the most recent one, the Chosen Pathway will revert back to the CHA/NI, whichever was completed last
