New Needs Identification

1.8 Create CCISD Case and Common Outcome Plan

Purpose

The new Needs Identification (NI) combines the previously existing Initial Contact-reporting purposes with the Needs Identification as was tested using the Common Holistic Assessment (CHA)-information collection under domains. The new NI can be done anonymously then attached to a registered individual, or simply completed and ‘filed’ for reporting. The new NI can also be done from an existing registered individual within the Common Outcome Plan (COP).

Procedure

Anchor Point: Person page

  1. Select the cases tab to determine if there is a CCISD Case and COP

    Method 1: CCISD Case Exists

  2. Go to the page action menu of the person page
  3. Select New Common Outcome Plan

  4. The page refreshes to the Common Outcome Plan page
  5. Under the Cases tab on the Person page there will now be a Common Outcome Plan

Method 2: CCISD Case and Common Outcome Plan Do Not Exist

  6. Go to the page action menu of the person page
  7. Select New Common Outcome Plan

  8. Under the Cases tab on the Person page there will now be a CCISD Case and Common Outcome Plan

Note: When you put an Anonymous NI to a registered file, whichever of the CCISD Case and Outcome     Plan that doesn’t already exist, will be auto created

Method 3: CCISD Case and Common Outcome Plan Exist

  9. Go to the page action menu
  10. The New Common Outcome Plan will be disabled because an open one already exists
  11. The New Case will not be disabled, even if a CCISD Case exists as this would impact Alberta Works in that they would be unable to create an Integrated Case. The system will not let the user create a second CCISD Case even though the button is still active
