New Needs Identification

1.5 Finish Anonymous Needs Identification

Purpose

The new Needs Identification (NI) combines the previously existing Initial Contact-reporting purposes with the Needs Identification as was tested using the Common Holistic Assessment (CHA)-information collection under domains. The new NI can be done anonymously then attached to a registered individual, or simply completed and ‘filed’ for reporting. The new NI can also be done from an existing registered individual within the Common Outcome Plan (COP).

Procedure

Anchor Point: Anonymous Needs Identification ‘Finish’ page

  1. Once the Anonymous NI is ready to finish, select the Finish button
  2. If any fields were not completed correctly, error messages will open. Correct the errors
  3. There are two options to proceed, select the option required



    Save Needs Identification Anonymously
  4. Select the check box for Save Needs Identification Anonymously if the Individual chooses not to be registered 



    Note: Once the Save Needs Identification Anonymously is selected, the information will not be retrievable and will not appear on that Individual’s file if they then choose to register
  5. Select the Next button and the Anonymous NI will be stored in the data base for reporting purposes

    Proceed to Search and register and Save NI
  6. Select the check box for Proceed to Search and Register and Save NI if the individual agrees to register, has previously been registered, or is in another data base and shows as not registered when searched
  7. Select the Next button

    Note: An NI contains the same fields regardless if it is Anonymous or within a Common Outcome Plan, and is completed following the same processes for completing the domain, needs, pathways, outcomes and next steps for the New Needs Identification
