Form 24: Consent of Proposed Guardian (Individual)

General Instructions

This document tells the Court that you consent to being a guardian and agree to act in accordance to the duties and responsibilities of a guardian as described in the Adult Guardianship and Trusteeship Act. The duties and responsibilities of a guardian are listed in Items 5 through 9 on this form. This form also states you are authorizing a Review Officer with the Office of the Public Guardian to conduct a criminal record check on you if it is needed for the Review Officer’s Report.

Court file number: Leave blank if this is for an initial application. A court file number will be assigned when the documents are filed with the court. If this is for a review of an existing order, refer to that order for the Court File Number. Do not write anything in the court stamp box below the file number box.

Judicial Centre: The application should be made in the Judicial Centre nearest to where the proposed represented adult (hereafter referred to as “the adult”) lives. Choose the Judicial Centre closest to where the adult lives from the choices provided. If you are completing the form on your computer, there is a drop down list of the available Judicial Centres. There is also a list provided at the end of these instructions. If you are unsure which Judicial Centre should be chosen, contact the Office of the Public Guardian. If this is for a review of an existing order, enter the Judicial Centre of where the existing order was granted.

Adult’s Full Name: Fill in full legal name of the adult. Do not use nicknames or abbreviations. Use the same name all the way through the documents. If this is for a review of an existing order, please enter the adult’s name as it appears on that existing order.

Applicant’s name: Fill in full legal name of the person making the application (this is usually the person who wants to become the guardian); do not use nicknames or abbreviations. Use the same name throughout the documents.

Each proposed “main” or “primary” guardian(s) must complete and sign their own individual Form 24 – Consent of Proposed Guardian (Individual). Please read all the sections carefully.


1. Fill in the name of the proposed guardian. Note that each proposed guardian must complete a separate Form 24.

2. You are confirming that you are over the age of 18.

3. You are confirming that you believe yourself to be a suitable person to act as guardian for the adult.

4. Check the appropriate box depending on your situation. You are confirming that you are unaware of any conflict of interest that exists or could potentially come into existence if you are appointed guardian or you are disclosing an existing conflict of interest or potential conflict of interest. If you check the second box, please explain the situation.

5.-9. Please read all sections carefully. These are the duties and responsibilities of a guardian as described in the Adult Guardianship and Trusteeship Act.

Date and sign the document in the presence of a witness and have the witness sign the document and print their name.
