This form is completed by anyone who has received notice that an application has been made for someone and the person believes that a hearing should be held in Court to determine if the application is appropriate.

You are required to provide information to the Court about why you think the matter should go before the Court for a hearing as opposed to a desk application. Please check off the appropriate box(es) and include your reasons and/or an explanation. If you require more room, add an additional piece of paper with your reasons.

Please read the statement about costs very carefully – you may have to pay costs if the Court feels your objections are frivolous (not serious or trivial) or vexatious (brought to annoy or cause trouble).

Where indicated, provide your contact details so the Review Officer and Court can contact you if necessary.

Sign and date the form and return it to the Review Officer as indicated.
