07 Service Plan

Create a Service Agreement

Service Agreements reflect the steps and or the actions that will be taken and the commitment of the individual and Service Manager to achieving the goal of the Service Plan. Conditions that must be in place before a Service Agreement can be created are:
  • The individual and Service Manager agree that the structure and content of the plan meet the needs and objectives of the individual.
  • The Service Plan status is Approved.

Anchor Point: Service Plan Home Page

  1. Select Service Agreements Tab.
  2. Select New.
  3. Select Next.
  4. Complete the mandatory fields.
  5. Select Save.
  6. The Service Agreement will display with a status of Issued.
  7. Select Edit from the List Action Menu for the Service Agreement line.
  8. Record the Date Accepted for Service Agreement.
  9. Select Save.
  10. The Service Agreement status is Accepted and the Service Plan status will display as Active.

Mandatory Fields

Issue Date
Defaults to current date, change to the date the Service Agreement is created.

Address
Enter Address of the individual.

Reason
Select from drop down menu.

Note
Service Agreements can also be recorded as Rejected or Cancelled by completing the respective section of the Service Agreement page.
