Create a Service Plan from Scratch
Anchor Point: Integrated Case Home Page
- From the individual’s Integrated Case Home Page, select the Service Plan Tab from the Navigation Bar.
- Select New.
- Complete the mandatory fields.
- Select Save.
- The Service Plan Home Page will open and there will be an information message “This service plan cannot be modified as a goal has not been set”.
- Select Edit Goal from the Tab Action Menu.
- Select the Goal from the drop down menu.
- Select Save.

The individual (Primary Client)
Expected Start
Default is the current date, change to match the expected start date of activity.
Expected End
Default is the current date, change to reflect the date the activity is expected to be completed by.
Expected Outcome
Select from the list using magnifying glass.
