Contact Log Fields

Explanation

Purpose

Address Change

Address was changed, note it here but make the actual address change in person information tab on person record under ‘contact details’. This includes updating an email address or phone number

Application

An application was given to the individual

Appointment Booking

Booked an appointment with a CM, program staff etc. Include who the appointment is with, and the date and time of appointment

ASC Contact

This should not be used. Any contact with an ASC will be recorded within the appropriate specific category (i.e. document submission, appointment booking, address change etc.)

Case Consult

Conversation with supervisor or co-worker on an individual’s circumstances and what should/could be done

CM Contact

This should not be used. Any contact with a case manager should be recorded within the progress of actions, or the Holistic Assessment

Document Submission

Submission of an application or supporting documentation and what you did with the documents (i.e. faxed application to AISH, returned original to individual)

General Inquiry

Registered person asks a question or requires information

Missed Appointment

Appointment was scheduled but individual cancelled or didn’t show up

Multi-Disciplinary Team

Multi-Disciplinary team met or is meeting. Actual content of the meeting notes goes into ‘team collaboration’ under support network tab.

No Contact

Record when attempts made to contact individual with no success. This should include what was done when contact couldn’t be made (i.e. changed pathway back to supported self-service)

Referral

Made contact with a community agency, program to a specific person. Include whom the referral was to and whom you talked to.

Urgent Need

Individual came in with specific urgent need. Record what the urgent need was and what was done to remedy the need (i.e. No food-gave individual food bank addresses)
