R15. Request for Training and Employment Services - LMTA UPDATE

Description
The Training and Employment Service Request (TESR) form defines the core individual-level administrative data to be collected and/or updated when a request for training and or employment services is received.  Use of the form (EMP 0016) is a best practice.  The mechanism used to obtain the data is optional but the collection/updating of the individual-level data is expected. 

The information collected on this form is required to meet provincial and federal performance measurement/reporting requirements that apply to training and employment service participants. Individuals who are not job-ready are not to be asked to complete this form.

Procedure:
Confirm that the individual consents to provide their personal information.  Verbal consent is acceptable. 

Signatures are required on the last page for:
  • Active Employment Insurance (EI) claimants seeking a recommendation to attend full-time contracted training or self-funded training programs, and/or
  • Individuals participating in one of the following training and/or employment services:
    • Alberta Job Corps
    • Exposure Course(s)
    • Job Placement (only when engaged in job shadowing, work experience or casual labour pool)
    • Alberta Settlement and Integration Program
    • Immigrant Bridging
    • Transition to Employment Services
    • Integrated Training
    • Self-Employment
    • Workplace Training


For Records Management purposes, record the Mobius ID # and Name in the top right hand corner of the last page if signed.  Signing the WCB Acknowledgement is a precondition of attendance in the above-mentioned training and employment services.  In the event an individual refuses to sign the WCB Acknowledgement, they will not permitted to participate.  Signed records are retained in accordance with the appropriate Records retention schedule.

If the TESR form has been used for data collection, review the form to ensure responses have been provided. 

No record found:

  1. Search and enter the individual-level data from either Pages 1 – 3 of the TESR into the person record in Mobius or collect the responses verbally (telephone or in-person conversation) and enter information directly into Mobius. 
    Social Insurance Number (SIN) – a valid Social Insurance Number is mandatory to receive an assisted employment and training service.
    1. A valid Social Insurance Number (SIN) is a nine-digit number required to work in Canada.
    2. SINs beginning with a ‘9’ are provided to temporary workers and/or temporary residents who are not Canadian citizens nor permanent residents. These temporary SINs have an expiry date which will appear on the Immigration documentation.
    3. An individual whose SIN begins with a ‘9’ must provide the Immigration documentation that indicates they are authorized to work in Canada (work permit) and proof that the SIN has not expired, to be eligible to receive assisted employment services. Details of pertinent documentation is to be recorded in Mobius.
    4. Individuals whose SIN begins with a ‘9’ must provide Immigration documentation that indicates they are permitted to attend training in Canada (study permit) before they can begin to participate in training. This includes EI claimants seeking a recommendation to attend training as a Feepayer.

Date of Birth  A Date of Birth is mandatory to receive an assisted employment and training service. 
Address  An address is required.  No fixed address is not acceptable. 
Preferred Language   The collection of the individual’s language of choice is collected as a requirement under the federal Official Languages Act.  Alberta is not bound by the federal legislation but has agreed to record the preference. The default has been set to English.   
Gender Gender X is used by an individual who does not identify as male or female, or has chosen to self-identify as “X” 
Marital Status Mandatory field in Mobius 
Number of Dependents The federal government’s definition differs from Alberta’s definition of a dependent.  Alberta has agreed to provide a numeric response based on the federal definition to reflect the number of dependents for LMTA participants, prior to starting a service plan.  Zero is an acceptable response for no dependents. 


Definitions for each of the following data fields are available:

  • Mobius Help Text and
  • On the TESR form.

Disability  A response is required.  Prefer not to report is an acceptable response. 
Indigenous  A response is required.  Prefer not to report is an acceptable response. 
Immigrant  If yes, a year of landing is required. 
Visible Minority   A response is required.  Prefer not to report is an acceptable response. 

 

Are you a Convention Refugee?  This question is a leading question to help determine if the Country of Origin needs to be collected. The Yes/No response is not recorded in Mobius. 
Country of Origin   Convention refugees are not able to return to their country of origin because of a well-founded fear of persecution. 

 
Education Screen

Highest Level of Education  Formal education completed.  Do not include courses like first aid, WHMIS, etc. 
Institution   Name of the educational institution 
Address Option   For education completed outside of Canada select “Out of country” from the drop down values 
Dates attended   Approximate dates are acceptable. 
Have you received government-funded training in the past 4 years?  This question is a leading question to help determine available employment and training options.  The Yes/No response is not recorded in Mobius. 


Employment Screen

Employment Status at Intake A response reflecting an LMTA participant’s employment status prior to starting a service plan is required. 
Current or most recent employment   Enter all details of the current (if employed) or most recent employment. Approximate dates are acceptable. 
Reason for Leaving   The best response that describes the reason for leaving the last employment.  If currently employed, a response is not required. 


Assessment Screen

Primary Source of Income   Enter this information on the appropriate Assessment screen (Employability or Service Needs Determination) 


Once the responses have been recorded electronically in Mobius, pages 1-3 of the TESR form, if used for data collection, may be shred.
