Outcome Planning

5.4 Goal Create

Purpose

The Goals functionality will capture the individual / family goals and the prioritization or sequencing of those goals based on professional advice from Alberta Supports staff and other partners.

Procedures

Anchor Point: Goals tab in Common Outcome Plan

  1. From the individual / family Common Outcome Plan Page, select the Goals tab from the Navigation Bar
  2. Select New Goal

  3. Select the Goal Category from the drop down list
    Note:  The categories match the CHA domain descriptions
  4. Select the Client
  5. Select the Priority box to indicate the Goal has been chosen as a priority (if applicable)
    Note:  Multiple Goals may be set as Priority and show a green indicator  on the Goals list
  6. Select the Planning Applicable box (if applicable)
    Note:  Only Goals set as Planning Applicable can have Actions linked to them. If a Goal is not set as Planning Applicable, they will not show when Actions are created, thus Actions cannot be linked to them
  7. Enter the Goal description
  8. Enter the Reason (if applicable)
    Note:  If “Other” was chosen as the Goal category, please fill in the reason box
  9. Select Save

Notes:

  • Once Saved, the Goal will appear on the Goals list
  • An individual / family can have multiple Goals
