Product Provider

P04. Modify Product Provider Registration - Change in Owner and/or Legal Operating Name

A Legal name change and/or change in ownership will require prior approval by the proper authority before being entered into Mobius. 

If the Product Provider Registration is being ended, and a new one is being created, IMAGIS information must be obtained prior to the creation of the new Product Provider Registration.  Once this is completed, the original Product Provider Registration can be ended.

The following outlines the process when an approved Product Provider has a change in owner and/or legal name that necessitates the ending of the original Product Provider registration and the registration of the new Product Provider, or the amendment of an existing Product Provider record.

Pre-condition(s)


The Product Provider is registered in Mobius and has a change in owner and/or legal name that necessitates the ending of the Product Provider registration and the registration of the new Product Provider, or the amendment of an existing Product Provider Record

Trigger
Authorized Official has been notified of a change in owner and/or legal name.

Authorized Official

Mobius Support

  • If information is missing, refers back to the originator.
  • If form/request contains all required information, updates Mobius as per instructions.
  • Notifies the originator of the completion of any Product Provider registration changes via email.

Authorized Official

  • If there are referrals, notifies the Plan Item Owner(s) to amend the Service Plan(s).

Plan Item Owner

  • Amends the Service Plan as appropriate.
  • Notifies the originator upon completion.

Authorized Official

  • If there are no active referrals, notifies Mobius Support via an electronic EMP 5520 – Requesting New or Amendment to Product Provider form (RNAPP).  to update Mobius (delete appropriate session(s)), end/delete appropriate program(s)/session(s)/service(s)), and end the original Product Provider record).

Mobius Support 

  • If the information is inaccurate or there are active referrals, refers request back to originator.
  • If all required information is accurate, and there are no active referrals, updates Mobius as per instructions.
  • Notifies the originator of completion via email.

    Note
    If the change in information affects IMAGIS (e.g., name/address/banking information), Mobius Support will advise the Authorized Official to update IMAGIS (F30 – Modify a Product Provider).

Business Rules/Criteria/Standards

IMAGIS is the source of truth for a legal operating name.

Before a Product Provider record can be ended, it is necessary to:

  1. Delete all active sessions. 
  2. End/delete all active programs/sessions/services.

Programs, services or sessions cannot be ended/deleted if there are active referrals.

A program, session or service can be ended if the start date has passed. If the start date is in the future, it can be deleted.
