Alberta Child Health Benefit (ACHB)

Change of Information

Changes in household information must be reported by completion of the AEHB3002 Alberta Child Health Benefit Change of Information form or reporting to an ACHB representative, and may include:
  • New address

  • Addition of child(ren) to the eligible household unit

  • Change of parent(s) or guardian(s)

  • Removal of an eligible child from the household unit

  • Loss of spouse/partner through separation or death

  • Marriage or cohabiting partner union within the benefit year

  • Addition/removal/change in Coordination of Benefit information
  • 18-19 year old dependant no longer in school or no longer living in parental home

    Note
    If an applicant marries/cohabitates within the current benefit year, the spouse/partner’s statistical information from
    the AEHB3002 Alberta Child Health Benefit Change of Information form is entered into the enrollment database; however, the family’s financial eligibility is not reassessed to reflect the new spouse/partner’s net income until the next date for annual renewal.

    If an eligible child moves to another parent/guardian on a temporary basis (e.g. summer vacation) there is no need to cancel original status and re-apply for new status. The HBC moves with the eligible child.
