MO-2018-21 - Personal Benefit Rates and Frequencies

INTENT

To assist with the cost of medical supplies when the medical supplies are essential to manage the client’s or dependent child’s condition.

POLICY

In order to receive a personal benefit, the eligibility criteria and conditions as outlined in the Personal Benefits section must be met.

Medical supplies are one-time use or disposable products that may be provided to assist an AISH client or their dependent child to manage a specific medical condition.

Examples of Medical Supplies include, but are not limited to, wound dressings, syringes and incontinence supplies when they are not available to the client from another program or source.

Diabetic supplies are covered under the Health Benefits Card and are not considered Medical Supplies under this policy.

Substantiation
The client provides:

  • A letter from a healthcare professional explaining the client’s medical condition, the expected benefit of the Medical Supplies and the duration that the supplies are needed.
  • A vendor quote for the Medical Supplies, itemizing the supplies and cost of each item.
    • The vendor quote should indicate the estimated monthly cost if the Medical Supplies are needed on an ongoing basis.

AISH Workers may request additional documentation if the AISH Worker deems it necessary.

Rate
Actual reasonable cost up to a maximum of $453.00 per month.
